What is this study about?

This study will look at the function of your heart, lung, liver, kidney, and brain. This questionnaire will help us understand your background related to your brain function.

What kind of commitment is involved?

This form will take about 10 minutes of time and will compliment other tests completed as part of the deep phenotyping study. You will complete this during your baseline visit and 2 years later at your end of study visit.
